Upgrades take effect immediately and you get prorated credit. Downgrades apply at the end of your current billing period so you keep full access to your features until then.
Each unique file you upload uses one doc credit for that month. Re-uploading an updated version of the same file counts as a new upload and uses another credit.

Yes. File size upgrades are a one-time purchase that permanently raises your per-document upload limit. Buying the 100 MB boost after the 50 MB boost simply sets your limit to 100 MB, not 150 MB.
